Understanding Lymphedema
Lymphedema is a chronic condition caused by impaired lymphatic drainage, leading to swelling, discomfort, and increased risk of infection — most commonly affecting the arms or legs after cancer treatment, surgery, radiation, or trauma. Left untreated, it can progress and significantly impact daily life.
The good news: lymphedema is no longer only “managed.” With modern supermicrosurgical techniques, many patients achieve significant, lasting improvement — and in early-stage cases, sometimes near-complete resolution.
Meet Dr. Muhammad Omar Afzal
Dr. Omar is a fellowship-trained plastic and reconstructive surgeon with subspecialty expertise in supermicrosurgery — a highly refined surgical discipline involving the connection of vessels smaller than 0.8mm under high-powered microscopy. This precision allows for reconstructive and lymphatic procedures that were once considered impossible.
- Fellowship training in Reconstructive microsurgery.
- Only trained Lymphatic Microsurgeon In Punjab
- 12+ years of experience.
- 200+ LVAs performed
- Member of World Society for Reconstructive Microsurgery.

Conditions We Treat
- Upper extremity lymphedema (post-mastectomy / breast cancer-related)
- Lower extremity lymphedema (post-gynecologic, urologic, or melanoma treatment)
- Primary (congenital) lymphedema
- Lipedema with lymphatic involvement
- Recurrent cellulitis due to lymphatic compromise
- Post-traumatic and post-radiation lymphatic injury
Our Treatment Approach
1. Comprehensive Evaluation
Every patient begins with a detailed history, limb measurements, and advanced imaging — including ICG lymphography and/or MR lymphangiography — to map lymphatic function and determine candidacy for surgery.
2. Lymphaticovenous Anastomosis (LVA)
A minimally invasive supermicrosurgical procedure that reroutes lymphatic fluid directly into the venous system, bypassing damaged pathways. Ideal for early-to-moderate stage lymphedema, performed through incisions often just a few millimeters long.
3. Vascularized Lymph Node Transfer (VLNT)
For more advanced cases, healthy lymph nodes are transferred from a donor site to reestablish lymphatic drainage at the affected limb — often combined with LVA for optimal results.
4. Debulking / Excisional Procedures
For patients with significant fibrofatty tissue accumulation, direct excision or liposuction-based debulking may be combined with physiologic procedures to restore limb contour and function.
5. Integrated Post-Surgical Care
Coordinated care with certified lymphedema therapists, compression garment fitting, and long-term monitoring to protect and maintain surgical outcomes.
Why Choose Supermicrosurgery
- Targets the underlying cause of lymphedema, not just the symptoms
- Minimally invasive with small incisions and faster recovery than traditional surgery
- Can reduce reliance on lifelong compression garments and manual lymphatic drainage
- Backed by growing long-term outcome data worldwide
- Personalized surgical planning based on each patient’s lymphatic anatomy and disease stage
Patient Journey
- Initial Consultation — Review of medical history, physical exam, and treatment goals
- Diagnostic Imaging — Lymphoscintigraphy, High Frequency Ultrasound
- Personalized Surgical Plan — LVA, VLNT, debulking, or a combined approach
- Surgery Day — Outpatient or short-stay procedure under microscopic guidance, done in Local Anaesthesia.
- Recovery & Rehabilitation — Guided compression therapy and physical therapy
- Long-Term Follow-Up — Ongoing monitoring of limb volume and lymphatic function
Frequently Asked Questions
Am I a candidate for lymphedema surgery?
Candidacy depends on the stage of your lymphedema, prior treatments, and imaging findings. Early-stage patients often see the best results, but many later-stage patients benefit significantly as well.
Is this surgery covered by insurance?
Many procedures are covered when medically necessary and properly documented. Our team can help verify your benefits.
What is recovery like?
Recovery varies by procedure — LVA typically allows a quicker return to normal activity than VLNT or debulking. Your surgical plan will include a personalized recovery timeline.
Will I still need compression garments after surgery?
Many patients reduce their reliance on compression significantly; some are able to discontinue it. Results vary based on disease stage and individual response.
